Is there a medical reason for premature ejaculation? And why wouldn’t he feel like he had an orgasm?

My boyfriend has had problems in the past a few times where he ejaculates very quickly. I don’t get upset with him, but I am lacking the pleasure I need. I am wondering if there is a medical reason he could be having this problem?

We haven’t had sex in a month for reasons I would rather not share, but he lasted literally 2 seconds when we had sex for the first time since then. I’m worried about him.

He also says that when he cums so quickly, he doesn’t really feel any pleasure by it. What does this mean?
